Direct Benefit Transfer (DBT) Consent - Terms and Conditions 

• I agree and confirm that I have voluntarily submitted my Aadhaar number to ICICI Bank for the purpose of linking my Aadhaar number to my account with NPCI mapper to enable me to receive Direct Benefit Transfer (DBT) benefits and hereby give my consent to the Bank:- (i) to establish my identity or address proof by Aadhaar based authentication or verification system (ii) share my Aadhaar details with UIDAI, NPCI, concerned regulatory or statutory authorities as may be required under applicable laws. (iii) Use my mobile number mentioned below for sending SMS alerts to me.

• I understand and agree that: (a) upon authentication, UIDAI may share with ICICI Bank information such as my demographic information including photograph which ICICI Bank may use as a proof of identity or proof of address for the above mentioned purpose;(b) my Aadhaar details (including my demographic information) shared by UIDAI will not be used for any purpose other than the purpose mentioned above or as per requirements of applicable law; (c) my biometric information will not be stored by the ICICI Bank.

• I hereby understand that my seeding of Aadhaar details will not be processed if : a) Aadhaar details provided by me is incorrect; b) authentication with UIDAI fails due to mismatch of my details with UIDAI; c) there is technical failure while authenticating Aadhaar details with UIDAI.

• I hereby declare that all the above information voluntarily furnished by me is true, correct and complete in all respects.

  *NPCI Mapping is a process of associating a bank with Aadhaar number which is facilitated by National Payment Corporation of India (NPCI) for Direct Benefit Transfer to the respective bank who have linked the Aadhaar number to a specific bank account for receiving DBT for which customer has given the consent.
